4th Annual Confident Parent Fair

This ticket provides free access to all activities, workshops, booths and additional resources provided here at the Confident Parent Fair. In addition, it also adds the participants into our database for upcoming programs and activity days. Please see the consent agreement below.

Consent agreement for: photography | participation | research in upcoming programs.
I agree that the organization's Personnel or Volunteers on behalf of the organization or other professional partnered with the organization has permission to use photographs for uses including but not limited to publicity, advertising and web content of me and my child and I understand no royalty or fee or other compensation will be payable to me.
I agree that the organization's Personnel or Volunteers on behalf of the organization or other professional partnered with the organization may interaction with me and my child.
I agree that the organization's Personnel or Volunteers on behalf of the organization or other professional partnered with the organization may observe, discuss, and retain information concerning my child for educational and scientific research purpose.
As a parent, legal guardian or acting on behalf of, affirm that I have been completely informed all the activities that the child/youth will participate. I understand the general structure of the activities/programs and do not need to be informed of each and every activity.

I hereby voluntarily release, forever discharge the community, the corporation, its officers, directors, employees, volunteers and agents from any and all claims, demands, or causes of action, which are connected with me and my child's participation in the programs or the use of the equipment and facilities.
I agree to pay for any and all medical expenses incurred and give permission to the doctor or health care professional to provide medical care if necessary.

By securing this ticket I confirm that I have fully informed myself of the contents of this Parental Consent and Release by reading it before securing the ticket. I warrant that I possess all the rights, powers, and privileges of a parent or legal guardian necessary to execute this agreement with binding legal effect.

This ticket is an agreement to partner with Love of Learning Institute and support it's Fair with promotion and advertisement to your network of participants, clients or customers. Please also see the consent agreement below.
